With the Forms, the process changes completely. A secure link is automatically sent via SMS directly to the patient's phone. The patient can complete their health questionnaire from home—no app to install, no account to create. When they arrive at the office, it's already done.

The answers are automatically integrated into his file on agenda.ch. No re-entry, no risk of loss or transcription error.

Automatically generated informed consent

This is one of the most sensitive points in physiotherapy: consent to treatment must be documented, signed, and archived. Until now, this often involved a separate document, printed, signed by hand, then scanned or physically filed.

With agenda.ch, once the patient's responses are received, the consent form is automatically generated from the data in the file, based on a default template from PhysioSuisse. It is ready to be archived or signed—digitally, without printing.

Forms designed for your firm

Every practice is different. This feature allows you to create fully customized templates: medical history, current treatments, contraindications, and therapeutic goals. You define the fields you need just once, and the template then applies to each new patient.
